Joseph Daniel O'Toole is killed in GTA Liberty City Stories by Mickey Hamfists, after the Leone family decided that he isn't trustworthy anymore.
Many myth hunters believe that his ghost still haunts his old club, Paulie's Revue Bar, which after the events of GTA Liberty City Stories became known as Luigi's Sex Club 7.
There is little evidence at the club or anywhere else in the game that O'Toole's ghost exists, so this myth is most likely false.
